Instructions for Customers electing to fill-in and submit the downloaded contracts from the Millennium Networks web site.

The downloadable contracts available on the web site may be downloaded and filled in by our customers or Millennium Networks will gladly do them for you.  If you choose to complete them you must insure every form field is filled out on the Master Agreement and the Attachment A Overview for the 1603 CTAC Coop or the Attachment A Overview Alcatel Products CTAC Coop programs.  The Master Contract, Attachment A, and your company's purchase order must also have a valid Contract Number to refer to.  We normally generate this number and put it on the contracts for you but it can be done by the customer as well using our rules. 

For the 1603 CTAC Coop this number is generated by adding your company initials and the number of calls to be contracted to: PL-060502-COOP-xxxx#

Example:

Millennium Networks Incorporated for five (5) CTAC Calls would be filled in as: PL-060502-COOP-MNI5  Your Purchase Order should use or refer to this same contract number for the part number on your purchase order form.

For the MNI-Alcatel CTAC Coop this number is also generated by adding your company initials and the number of calls to be contracted to: PL-060502-COOP-xxxx#.  Your Purchase should reference this number and the part number selected in the MNI-Alcatel CTAC COOP Overview.

Example:

Millennium Networks Incorporated for five (5) CTAC Calls would be filled in as: PL-060502-COOP-MNI5 selected Part# NSD-MNICTAC-2

And, your purchase order would reference Agreement# PL-060502-COOP-MNI5 and Part# NSD-MNICTAC-2.

After completing the forms and signing the master agreement, and preparing your Purchase Order, you may email a pdf or scanned copy, fax, or mail it to us for processing.  We will counter sign the documents, copy them and send the counter signed copy back to you.  After the contracts are in place and payment received you will receive an email with instructions and phone numbers for support.
